504 Eligibility for Students with Diabetes

Diabetes can cause blood glucose (sugar) levels to be too high or too low, both of which affect the

student’s ability to learn as well as seriously endangering the student’s health both immediately and in the long term. The goal of the 504 Plan is to assure the student with diabetes has the same access to education as other children. It is a tool that can be used to make sure that the student, the parents/guardians, and school personnel understand their responsibilities and work through challenges or misunderstandings to avoid problems in the future.

Under Section 504 of the Rehabilitation Act of 1973 and Ohio House Bill 264 your student is entitled to a 504 Plan through Alliance City Schoolsto ensure your student’s safe participation in our schools programs – both academic and extracurricular.

A Section 504 Plan enables theAlliance City Schools to offer health accommodations and other support to assist your child while at school and during school related activities. This plan also can be used to make sure that the student, the parents/guardians, and school personnel understand each of their responsibilities to ensure your child’s safety. A 504 Plan is an option for parents and their child, not mandatory.
